Patient Forms
All new patients must complete their required forms at least 3 days before their scheduled appointment. If your appointment is within the next 3 days, forms must be submitted within 8 hours of booking.

Insurance Information
To verify your dental benefits, we require accurate insurance information at the time of booking. If additional details are needed, we will contact you.
If we do not receive the correct insurance information promptly, we reserve the right to cancel your new patient appointment.

X-Ray Policy
- If your insurance does not cover X-rays due to frequency limitations, you must either provide recent X-rays from another office or pay out-of-pocket for new X-rays at Oxford Dental.
- We cannot proceed with your appointment unless we have current X-rays on file at the time of your visit.
Cancellation & No-Show Policy
Cancellation & No-Show Fees:
- First missed appointment: Cancellation fee is waived as a courtesy.
- Second missed appointment: Cancellation fee is $75.
- Third consecutive missed appointment: Prepayment required.

Yes, but the same 24-hour policy applies. If rescheduling is requested less than 24 hours before your appointment, a cancellation fee may be applied.
